Job Overview

  • Salary: $16.00 – $19.00 per hour
  • Schedule: Full-time (40 hours/week), Monday to Friday, typically 9:00 AM โ€“ 6:00 PM EST

Primary Responsibilities

  • Conduct routine monthly engagement phone calls with patients diagnosed with chronic conditions.
  • Monitor incoming remote physiological data (such as blood pressure, pulse ox, and weight measurements) to track stability.
  • Identify and escalate critical health concerns or reading spikes in real time to the clinical Nurse Manager.
  • Maintain a target efficiency of 90% when tracking and documenting care coordination time within Healthmote and EHR software platforms.
  • Actively encourage patients to utilize their remote monitoring equipment consistently to ensure seamless data streams.

Eligibility Criteria

To qualify for this position, candidates must hold an active Medical Assistant Certification from an accredited body. Because this role requires direct communication with vulnerable patient groups, a minimum of 2 years of experience as a Medical Assistant in a professional healthcare or clinical environment is strictly required. High school diploma or equivalent is highly preferred to align with our clinical onboarding documentation standards.

Candidates must exhibit an expert-level understanding of medical terminology, basic pharmacology, and standard vital sign thresholds. Since this is a fully remote position, exceptional computer literacy, confidence navigating complex electronic medical records (EMRs), and the ability to work independently are absolutely essential to succeed.

Preferred candidates will bring a minimum of 1 year of specific experience within Chronic Care Management (CCM), Remote Patient Monitoring (RPM), or Transitional Care Management (TCM). Experience communicating across diverse, culturally rich, or multilingual populations will also make your application stand out significantly during our review process.
