Primary Responsibilities

  • Serve as a vital link between complex-needs patients, primary care physicians, and clinical case management teams.
  • Actively engage targeted patient populations through multiple channels, including phone calls, text messages, and patient portals to schedule high-risk check-ups.
  • Seamlessly close care gaps, navigate specialty care referrals, and connect patients with vital community-based organizations.
  • Document accurate patient interviews, medical histories, and health issues in real-time within major Electronic Health Records (EHR) software like Epic, Athena, and Cerner.
  • Conduct occasional travel (10% to 15%) to regional clinical sites in the Atlanta area to manage provider engagement and perform retinal eye exams.

Eligibility Criteria

To be considered for this competitive clinical navigation role, candidates must hold an active certification or registration. Guidehealth requires a Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), or a Certified EMT credential, backed by a minimum of 2 years of hands-on healthcare experience. Strong foundational knowledge of medical terminology, primary care workflows, and the social determinants of health is completely essential for success.

Technically, applicants must demonstrate a high proficiency in navigating multi-platform systems, including the Microsoft Office Suite (Outlook, Word, Excel) and various EHR databases. Because this is a patient-facing communication position, exceptional critical listening, verbal clarity, and empathetic conversation skills are absolute must-haves.

A dedicated, private home office free from ambient noise or distractions is mandatory to ensure strict patient confidentiality. Candidates must also possess a valid state driver’s license, reliable personal transportation for field travel, and an internet connection meeting a minimum threshold of 100 Mbps download and 10 Mbps upload speed.
