Primary Responsibilities:

  • Referral Management: Schedule appointments with external service providers in the PACE network and manage electronic health records.
  • DME Coordination: Procure durable medical equipment (DME) and supplies according to medical orders.
  • Participant Support: Communicate with participants, families, and caregivers to ensure appointment attendance and equipment needs are met.
  • Documentation: Maintain precise logs of all DME orders and referral-related communications within the electronic health record system.

Eligibility Criteria:

The ideal candidate will have strong administrative skills and a clear understanding of medical documentation. You should be detail-oriented, comfortable navigating electronic health records, and able to communicate effectively with vendors and medical staff. Proficiency in coordinating complex schedules and familiarity with durable medical equipment are essential for success in this role.

While specific prior experience in a PACE program is highly preferred, a background in medical records, administrative support, or patient coordination will be considered. We value applicants who demonstrate empathy, patience, and a strong sense of urgency when addressing the needs of our older participants.

You must be prepared to manage multiple priorities simultaneously, including faxing/submitting referrals, communicating with transport services, and assisting the reception desk as needed. Your role is critical to the daily operation of our health program and the quality of life of those we serve.
