Primary Responsibilities
- Build trusting relationships with targeted, high-risk patient populations via remote bi-directional communication channels.
- Identify complex medical, behavioral, social, emotional, and financial needs to ensure comprehensive, whole-person care coordination.
- Educate and support patients in following personalized care plans, using motivational interviewing to bridge gaps in preventive health.
- Facilitate seamless communication between patients and primary care physicians, managing referrals, scheduling, and community resource links.
- Accurately document all care interactions, non-clinical assessments, and task reconciliations within modern Electronic Health Record (EHR) platforms.
Eligibility Criteria
To qualify for this vital role, candidates must meet the foundational educational and credentialing standards. Guidehealth prefers a Certified Medical Assistant (CMA), a Certified Community Health Worker/Assistant, a Licensed Practical Nurse (LPN), or a Certified EMT. Alternatively, a Bachelor’s Degree in a healthcare-related field alongside relevant clinical experience is highly valued. A Patient Navigation certification is a strong plus, or you must be willing to earn one within your first year of employment.
Fluency in both English and Hindi is strictly required to effectively serve bilingual patient populations. Candidates must possess at least 2 to 3 years of hands-on experience in patient navigation, care coordination, medical assisting, or health promotion. Equivalent years of undocumented, direct patient-facing experience with complex medical or social needs will also be considered. Proficiency with major EHR systems like Epic, Athena, or Cerner, along with high-speed remote internet capabilities (minimum 100 Mbps download / 10 Mbps upload), is required.
